Please be specific with the symptoms your child is experiencing. Remember, absences for travel are unexcused unless the Principal gives a special exception in advance. If your child arrives to school after 9:00am, they must sign in at the health office and will be marked tardy. Please try to avoid scheduling routine medical, dental, and orthodonture appointments during the school day.
Policy on Medications in School The Board of Education of Regional School District 15 policy complies with the state of Connecticut’s health regulations. 1. Medications may only be administered in school with the written order of a physician or dentist, and written authorization of the parent/guardian. 2. Doctor’s orders are needed for all medications including Tylenol or Advil and other over-the-counter medications. A form can be obtained from the nurse if a medication is required during school hours or printed off this website. 3. Students may self-medicate with doctor’s order. 4. Medications must be brought to the school nurse by a parent or other responsible adult and must be in the original container. Students are not permitted to transport medication unless they also have an order to self-administer the medication. 5. New medication orders must be written for every school year. 6. If your child is on medication during the school day and is going to need medication during an extended field trip, you, the parent/guardian, must contact the school nurse one week in advance of the trip to make arrangements for the administration of the medication. Kindergarten Registration: All forms for Kindergarten Registration can be found on the Region 15 website under Registration tab. The State of CT Physical form (completed by the physician) must have all “starred areas” completed and sent to the school as soon as possible. The exam must have done after 7/31/09 for the 2010/2011 school year.
Upcoming 6th Grader’s (fall of 2010)
Current 5th graders must have a complete physical that is performed on or after July 1, 2010 on the State of CT form. This must be on file in the Rochambeau nurse's office by February 15, 2011.
